Tottenham Hotspur's chances of staying in the Premier League have suffered a fresh blow, with Mohammed Kudus potentially facing surgery to address a persistent quad problem.

As the top flight prepares to resume this weekend, Spurs sit just one point above the relegation places with seven matches remaining. They will drop into the bottom three for the first time in 18 years should West Ham United defeat Wolverhampton Wanderers on Friday evening.

Spurs have turned to the highly regarded Roberto De Zerbi to drag them out of trouble following Igor Tudor's disastrous 44-day caretaker spell, and the Italian's arrival should provide a significant lift to a fragile Spurs side. Reports indicate the players have been thoroughly impressed with their new head coach thus far.

Kudus Could Miss the Remainder of Tottenham's Wretched Season

With De Zerbi appointed and James Maddison progressing in his recovery from an ACL injury, things appeared to be looking up for Tottenham. Furthermore, Tudor had indicated before his exit that Kudus was set to return following the international break, having been sidelined since early 2026 with a quad complaint.

The Ghanaian winger had been a driving force for the Lilywhites during Thomas Frank's troubled tenure, contributing seven Premier League goal involvements in the opening half of the campaign. Notably, Spurs have failed to win a single league fixture since he limped off against Sunderland on January 4.

As a result, fans were eagerly anticipating his return to the fold. However, rumours of a setback began circulating on Thursday, and those reports were subsequently confirmed by the club.

Spurs confirmed that Kudus had resumed training over the previous week after more than three months on the sidelines, but will now consult a specialist to assess whether an operation is necessary. Should surgery be required, Kudus would not only miss the rest of the domestic campaign, but his involvement at the World Cup could also be thrown into serious doubt.

Kudus' injury came after Spurs had already offloaded Brennan Johnson to Crystal Palace. The severity of his absence meant the club were widely expected to strengthen in the January transfer window, but those at the helm in north London — namely Johan Lange and Vinai Venkatesham — chose not to reinforce Spurs' attacking options.

Sporting director Lange explained that the club deliberately avoided "panic buys," believing that several injured players would soon be fit to return.

Regardless of the reasoning, both Frank and Tudor were left with a depleted squad. Now, De Zerbi must find a way to compensate for Kudus' considerable absence. Natural right-wingers are in short supply at Spurs, particularly following Wilson Odobert's ACL injury in February, leaving only Randal Kolo Muani, Djed Spence (primarily a full-back) and Pedro Porro (also a full-back) as realistic alternatives.

The Italian will need to think outside the box, as there is no straightforward answer. Kudus had been widely tipped as the player to carry Spurs through the final stretch of the season, but a new saviour must now step forward in Lilywhite.
